Advance Care Planning consultations are covered by National Health Insurance (NHI) for individuals with the following qualifications. For other cases, please refer to the latest announcements on our hospital website.
(1) Patients aged 65 and above with a major illness recognized by NHI.
(2) Patients who meet the eligibility criteria for hospice and palliative care.
(3) Patients with a Clinical Dementia Rating (CDR) score of 0.5 to 1.
(4) Patients announced under Subparagraph 5, Paragraph 1, Article 14 of the Patient Right to Autonomy Act.
(5) Patients enrolled in the “National Health Insurance Home Medical Care Integration Program.”
(6) Patients aged 65 and above with multiple chronic conditions who are enrolled in the “National Health Insurance Family Physician Integrated Care Program” or the “National Health Insurance District Hospital Whole-Person and Community Care Program.”
In addition to the patient, at least one relative within the second degree of kinship must attend the consultation. If a medical power of attorney has been designated, the appointed healthcare agent must also be present.
With the patient’s consent, other family members may attend the consultation. Due to space limitations, the total number of attendees must not exceed six.
The consultation will be held at the Conference Room of the Community and Home Care Center, 2nd Floor, NTUH Jinshan Branch.
Each consultation lasts approximately 60 minutes. Check-in times are as follows:
Morning: Session 1 – 09:00, Session 2 – 10:00, Session 3 – 11:00
Afternoon: Session 1 – 13:30, Session 2 – 14:30, Session 3 – 15:30
